Checking your coverage

Insurance plans change what they cover, and networks change every year. The surest way to know what to expect is to let us check for you before your visit. When you request an appointment, have your insurance card ready; our staff will confirm your plan when they call you back.

At your visit

Bring your insurance card and a photo ID. Copays and any amount toward your deductible are collected at the time of your visit.

If you have a question about a statement, call the office during business hours and we will go through it with you.
