Welcome. Here is what to expect at your first visit, so that nothing about it is a surprise.

Before your visit

Request an appointment online or call the office. A member of our staff will phone you to confirm a date and time; an appointment is not booked until you hear from us. If you need to cancel or move it, please call as early as you can so the time can go to someone else.

What to bring

  • Your insurance card and a photo ID
  • A list of the medications you take, including over-the-counter ones
  • The shoes you wear most, and any inserts or braces you use. They tell us a great deal.
  • Any recent imaging or reports from another doctor, if you have them

Arriving

Plan to arrive about fifteen minutes early to complete your paperwork. If you have a concern about getting from your car to the office, call ahead and we will help.

During the visit

Dr. Hasmukh will ask about your history and what the problem keeps you from doing, examine your foot or ankle, and use imaging if it is needed. She will explain what she finds and your options in plain language. Many problems can be treated at the same visit.

After the visit

You will leave with a clear plan and written instructions. If anything changes before your follow-up, call the office. Do not wait.

New patient forms

Filling these out before your visit saves time in the waiting room. Download the form, fill it in on your phone or computer, then print it or save it and bring it with you. If you would rather fill it in by hand, print it blank and bring it in.

Please bring the completed form with you. Do not email it. Email is not a secure way to send medical information.
